Requirements

Candidates should possess a Bachelor's degree in Social Work, Healthcare Administration, or a related field, and have a minimum of 3 years of experience in case management, preferably within the child welfare or managed care environment. Strong knowledge of Medicaid regulations, child welfare policies, and trauma-informed care practices is required, along with excellent communication and interpersonal skills.

Responsibilities

The CFSP Foster Care/Adoption Care Manager will provide integrated whole-person Care Management under the CFSP Care Management model, coordinating across physical health, behavioral health, I/DD, LTSS, pharmacy, and unmet health-related needs. They will collaborate closely with County Child Welfare Workers to align health care needs with permanency planning goals, conduct assessments, and oversee non-clinical needs of members with chronic illnesses, co-morbidities, or disabilities, and work with a multidisciplinary care team to coordinate care and identify members with potential clinical health care needs.
