Requirements

Responsibilities
The CFSP Foster Care/Adoption Care Manager will provide integrated whole-person care management under the CFSP Care Management model, coordinating across various healthcare services including physical health, behavioral health, I/DD, LTSS, and pharmacy. They will conduct assessments, collaborate with county child welfare workers and multidisciplinary teams, identify members with clinical needs, and oversee non-clinical needs of members with chronic illnesses. Additionally, the role involves coordinating care with juvenile justice stakeholders and offering trauma-informed care to support children’s diverse needs, aligning health care needs with permanency planning goals.
