Responsibilities

  • Lead the clinical team in providing the best patient care and creating a great place to work
  • Practice primary care in clinics alongside provider colleagues at 80% clinical time and 20% administrative time
  • Set an example for thoughtful primary care practice
  • Serve as a formal mentor, coach, and advisor to providers in Oak Street clinics
  • Participate in critical organization-wide strategic initiatives
  • Work in collaboration with the Practice Manager and Outreach Manager/Director to achieve organizational objectives
  • Drive implementation of Oak Street Health (OSH) strategic initiatives
  • Follow direction from regional leadership to implement regional priorities and reach regional objectives
  • Cascade organizational information to care team members and escalate care team concerns and ideas
  • Help coordinate and lead the onboarding of new providers at the center
  • Model excellent medical care, implementation of the care model, and success in metrics
  • Support consistent and robust OSH care model implementation, including daily huddles, patient care planning and management, admission prevention work, and clinical documentation workflows
  • Ensure medical care provided by each provider and care team is appropriate evidence-based care that keeps patients healthy and out of the hospital
  • Understand, teach, and implement Oak Street’s clinical guidelines at the individual provider and center level
  • Support each care team and the center to provide the highest quality primary and secondary care
