Responsibilities
- Collaborate with the Practice Manager and Outreach Manager/Director to achieve organizational objectives
- Implement OSH strategic initiatives
- Cascade organizational information to care team members and escalate concerns to the organization
- Coordinate and lead the onboarding of new providers
- Model excellent medical care and implementation of the care model
- Ensure consistent and robust OSH care model implementation
- Support each care team to provide the highest quality (evidence-based) care
- Understand, teach, and implement Oak Street’s clinical guidelines
- Provide appropriate evidence-based care
- Ensure that the medical care provided by each provider and care team is appropriate
