Responsibilities

  • Collaborate with the Practice Manager and Outreach Manager/Director to achieve organizational objectives
  • Implement OSH strategic initiatives
  • Cascade organizational information to care team members and escalate concerns to the organization
  • Coordinate and lead the onboarding of new providers
  • Model excellent medical care and implementation of the care model
  • Ensure consistent and robust OSH care model implementation
  • Support each care team to provide the highest quality (evidence-based) care
  • Understand, teach, and implement Oak Street’s clinical guidelines
  • Provide appropriate evidence-based care
  • Ensure that the medical care provided by each provider and care team is appropriate
