Responsibilities

Collaborate with the Practice Manager and Outreach Manager/Director to achieve organizational objectives
Implement OSH strategic initiatives
Cascade organizational information to care team members and escalate concerns to the organization
Coordinate and lead the onboarding of new providers
Model excellent medical care and implementation of the care model
Ensure consistent and robust OSH care model implementation
Support each care team to provide the highest quality (evidence-based) care
Understand, teach, and implement Oak Street’s clinical guidelines
Provide appropriate evidence-based care
Ensure that the medical care provided by each provider and care team is appropriate
