Requirements

  • Resident of Texas Mid-Cities area (NE Fort Worth to NW Dallas, including Keller, Grapevine, Euless, Coppell, N. Irving)
  • Registered Nurse – Texas or Compact license
  • 3+ years clinical practice experience in Pediatrics
  • 1+ year Case Management experience
  • Must possess reliable transportation and be willing and able to travel up to 75% of the time (mileage reimbursed per company policy)
  • Associates Degree required (BSN preferred)

Responsibilities

  • Telephonically and/or face-to-face assess, plan, implement, and coordinate all case management activities with members to evaluate medical needs and facilitate overall wellness
  • Develop a proactive course of action to address issues, enhance short- and long-term outcomes, and promote member wellness through integration of services, strategies, policies, and programs
  • Coordinate and implement assigned care plan activities and monitor care plan progress
  • Conduct multidisciplinary reviews to achieve optimal outcomes and identify/escalate quality of care issues through established channels
  • Utilize negotiation skills to secure appropriate options and services to meet member benefits and/or healthcare needs
  • Utilize influencing/motivational interviewing skills to ensure maximum member engagement and promote lifestyle/behavior changes for optimum health
  • Provide coaching, information, and support to empower members to make independent medical and healthy lifestyle choices
  • Help members actively and knowledgeably participate with their provider in healthcare decision-making
  • Utilize case management and quality management processes in compliance with regulatory, accreditation guidelines, and company policies/procedures for monitoring, evaluation, and documentation of care
