Requirements

Resident of Texas Mid-Cities area (NE Fort Worth to NW Dallas, including Keller, Grapevine, Euless, Coppell, N. Irving)
Registered Nurse – Texas or Compact license
3+ years clinical practice experience in Pediatrics
1+ year Case Management experience
Must possess reliable transportation and be willing and able to travel up to 75% of the time (mileage reimbursed per company policy)
Associates Degree required (BSN preferred)

Responsibilities

Telephonically and/or face-to-face assess, plan, implement, and coordinate all case management activities with members to evaluate medical needs and facilitate overall wellness
Develop a proactive course of action to address issues, enhance short- and long-term outcomes, and promote member wellness through integration of services, strategies, policies, and programs
Coordinate and implement assigned care plan activities and monitor care plan progress
Conduct multidisciplinary reviews to achieve optimal outcomes and identify/escalate quality of care issues through established channels
Utilize negotiation skills to secure appropriate options and services to meet member benefits and/or healthcare needs
Utilize influencing/motivational interviewing skills to ensure maximum member engagement and promote lifestyle/behavior changes for optimum health
Provide coaching, information, and support to empower members to make independent medical and healthy lifestyle choices
Help members actively and knowledgeably participate with their provider in healthcare decision-making
Utilize case management and quality management processes in compliance with regulatory, accreditation guidelines, and company policies/procedures for monitoring, evaluation, and documentation of care
