What You’ll Do

The Care Team Assistant provides essential clerical support and ensures the delivery of high-quality, compassionate, evidence-based care within a Value-Based Care environment. Your responsibilities will include:

  • Accepting incoming phone calls and returning calls from patients regarding clinical inquiries for assigned providers.
  • Performing protocol-based triage and routing phone calls via phone note documents in the EHR to providers for review and instructions.
  • Placing calls and creating template-based letters, as directed by providers, for timely delivery of lab results to patients.
  • Reviewing daily schedules to ensure all pre-visit preparations are complete, including the availability of lab and imaging results, consultation reports, care transition details, or significant health status changes in the patient's medical record.
  • Contacting patients, consultants, imaging centers, and/or hospitals to arrange for the timely delivery of necessary documents when they are missing from the patient's record.
  • Calling patients identified as at risk for a "no-show" and new patients for their onboarding visit to remind them of responsibilities.
  • Contacting "no-show" patients to understand their status, need for rescheduling, assess barriers to care, and initiate appropriate social service referrals.
  • Monitoring and responding to questions and escalations within CareHub and other internal platforms.
  • Reviewing TOC lists and assisting in determining discharge dates.
  • Contacting patients to schedule follow-up appointments.
  • Monitoring and responding to questions and escalations in Teams chats.
  • Assisting the care team with regular report completion for population health management and assessment.
  • Identifying patients belonging to "vulnerable populations" and providing appropriate social service or community-based referrals.
  • Assisting care teams and coordinators with data tracking and caseload tracking.
  • Collaborating virtually with colleagues and care team members.
  • Performing other duties as assigned.

What You Bring & How You Qualify

We are seeking individuals passionate about reimagining pediatric healthcare and committed to ensuring every child with complex medical conditions receives the care and support they deserve. To be successful in this role, you will need:

  • High-school level education or equivalent.
  • A minimum of 2 years of clinical experience in a behavioral health inpatient, outpatient, ambulatory clinic, or virtual setting, with experience in record management and requests.
  • Preferred working knowledge of the hospital setting and discharge planning processes.
  • Medical Assistant Certification is preferred.
  • Spanish fluency is preferred.
  • Demonstrated competency with an EHR system.
  • This position requires access to confidential and sensitive information, necessitating ongoing discretion and secure information management.

What We Offer (Benefits + Perks)

The target hourly rate for this position starts at $21-24 per hour. In addition to a competitive company benefits package, you will be eligible to participate in an employee equity purchase program (as applicable). Compensation will be determined by analyzing and carefully considering job-related knowledge, skills, and experience.
