Bilingual (Spanish) Transitions of Care Nurse Practitioner

Candidates must possess an active Nurse Practitioner (NP) or Physician Assistant (PA) license, along with at least two years of telehealth experience and significant experience in mental health. They must hold multiple state licenses, be able to work at least 16 weekend hours per month, and be willing to work four holidays and holiday weekends annually. Applicants should have excellent written and verbal communication skills, no restrictions to be credentialed with Medicaid or other payers, and a passion for patient education. Fluency in Spanish is a plus.
The Nurse Practitioner/Physician Assistant will consult and treat patients experiencing various conditions, proactively check in with patients on their treatment plans, collaborate with customer service for operational support, respond to patient messages and adjust treatment plans accordingly, and provide feedback to improve quality of care and operational processes. They will also deliver empathetic, high-quality care and perform other duties as assigned.
