Requirements

Candidates must possess an active Nurse Practitioner (APRN) license in good standing in the required states or be able to obtain licensure in multiple states within 90 days of hire. Board certification in Family Medicine, Adult-Gerontology, or Internal Medicine is required, along with a minimum of 2 years of primary care, urgent care, or internal medicine experience, preferably with telehealth or virtual care experience. Active BLS certification at the time of hire is also necessary, and flexibility with shift times, including potential future weekend shifts, is expected.

Responsibilities

The Virtual Primary Care Nurse Practitioner will provide comprehensive virtual primary care visits, including assessments, chronic disease management, preventive screenings, and acute care interventions. Responsibilities include diagnosing and managing chronic conditions, evaluating and treating acute concerns, ordering and interpreting diagnostic tests, and managing medication prescriptions. The role also involves providing health coaching, patient education, identifying patients needing referrals or home-based services, and coordinating care with specialists or community resources. Additionally, the Nurse Practitioner will utilize and provide feedback on the electronic health platform, participate in clinical case reviews and interdisciplinary collaborations, and engage in continuous learning to stay updated on best practices in virtual and primary care.
