Ability to provide mental health resources and therapeutic interventions using evidence-based approaches
Expertise in working with individuals living with Serious Mental Illness, Substance Use Disorder, experiencing homelessness, and/or high medical needs
Willingness to conduct 1-2 on-site community visits per month alongside team members (90% remote work)

Responsibilities

Primarily work with and support enrolled individuals who struggle with severe mental illness or substance use disorders
Use evidence-based approaches to provide support, mental health resources, and therapeutic interventions
Collaborate in a team-based model with lead care manager, community health worker, registered nurse care manager, and nurse practitioner
Contribute behavioral health expertise to improve individuals' quality of life through evidence-based behavioral interventions
Participate in 1-2 monthly on-site visits in the community close to your city (while majority of work is remote)
