Responsibilities
- Serve as a liaison between healthcare services and Devoted members
- Work closely with clinicians to provide high-touch telehealth visits
- Identify care gaps and coordinate across a fragmented healthcare system
- Communicate and work with PCP offices, pharmacies, and home health care providers
- Coordinate durable medical equipment (DME), home health care, imaging, and laboratory testing needs
- Help manage member adherence to a plan of care
- Prepare members for telehealth visits and ensure they feel comfortable and at ease
- Order management and care coordination, including same-day coordination
- Work with home health partners to coordinate DME and skilled nursing services
- Coordinate lab testing and imaging, and track results to ensure review by the ordering clinician
- Notify members of lab results and help coordinate follow-up care
- Coordinate with PCPs on medical record collection, information gathering, and appointment setting
- Follow up with members post-hospital discharge to ensure a smooth transition plan
- Triage clinical and non-clinical issues from members to the appropriate teams
- Escalate behavioral health-related calls to the Devoted Medical Behavioral Health Team
- Educate members on existing Devoted clinical programs and offer internal referrals
- Support member portal communication
- Serve as an advocate for members when they call Devoted Medical for care coordination issues
