Responsibilities

Pre-Visit Prep: Preparing members to feel comfortable with video onboarding prior to telehealth visits
Post-Visit Prep
Order Management/Care Coordination (same-day): Coordinating DME and skilled nursing with home health partners; coordinating lab testing and imaging, tracking results for clinician review; notifying members of lab results and coordinating follow-up care; coordinating with PCPs on medical records, appointments, and communications
Following up with members post-hospital discharge to ensure smooth transitions and support HEDIS stars measures
Triage Support (same-day): Triaging clinical and non-clinical issues to appropriate teams (clinicians, practice management, high-risk nurse, social worker); escalating behavioral health calls to the Behavioral Health Team
Patient Education: Advocating for members on care coordination calls; educating on Devoted clinical programs (e.g., Community Guides, HTN program) and offering internal referrals; supporting member portal communication
Flexible contributions across Specialty Care, Care OnDemand, Devoted to Me programs as needed
Occasional weekend coverage through optional overtime
