Willingness to conduct 1-2 on-site community visits per month alongside a team member, near your city (position is primarily remote flexible, ~90% remote)
Experience or ability to work with individuals living with Serious Mental Illness/Substance Use Disorder, experiencing homelessness, and/or high medical needs
Highly motivated, creative, and empathetic thinker

Responsibilities

Maintain ongoing caseload of individuals through utilization of evidence-based approaches to promote engagement and achievement of health goals
