Must reside in or near Hialeah, Florida (Applicable Zip Codes: 33010, 33012, 33013, 33014, 33015, 33016, 33018, 33142, or 33147)
Willing and able to travel 50-75% of time for face-to-face visits with members in Miami Dade county and surrounding areas in FL
1+ year of experience in behavioral health or a long-term care setting
Bachelor's degree required, preferably in social work or related field
Computer proficiency in Microsoft Word, Excel, and Outlook

Responsibilities

Telephonically and/or face-to-face assess, plan, implement, and coordinate all case management activities with members to evaluate medical needs and facilitate overall wellness
Effectively manage a caseload that includes supportive and medically complex members
Develop a proactive course of action to address issues presented to enhance short and long-term outcomes and overall wellness through integration
Determine appropriate services and supports due to member's health needs, including but not limited to: Prior Authorizations, Coordination with PCP and skilled providers, Condition management information, Medication review, Community resources and supports
