Candidates must reside within 2-3 miles of zip codes 33624 or 33625 in Tampa, FL, and the role requires 50-75% travel for face-to-face visits with members in Hillsborough County or surrounding counties. The schedule is Monday-Friday, 8:00am-5:00pm.

Required qualifications include residing in zip code 33624 or 33625 (or within 2-3 miles), being bilingual in English and Spanish, willingness to travel 50-75%, 1+ years of experience in behavioral health or long-term care, computer proficiency in Microsoft Word, Excel, and Outlook, and a Bachelor's Degree in Social Work or related field.

The position is full-time with anticipated 40 weekly hours, Monday-Friday 8:00am-5:00pm, and promotes autonomy with paid holidays and flexibility in coordinating member care.
