  • Family, Adult, or Adult-Gerontology Social Worker who has graduated or will graduate from MSW program
  • Willing to work in assigned location (Tulsa, OK) for program duration
  • Minimum GPA of 3.0 in MSW program
  • Holds or will hold by fellowship start date an active, non-probationary state lower level social work license in working state
  • Application attachments: CV with work history and social work internships details, completed application, MSW Diploma (if available), unofficial graduate school transcript, copy of current Social Work license (if available), proof of work eligibility for non-US residents (if applicable)

Responsibilities

  • Participate in 12-month paid training program in collaborative care model in value-based setting
  • Build knowledge of evidence-based interventions and psychotropic medications
  • Earn clinical supervision hours from a licensed clinical social worker
  • Engage in didactic content building on graduate training
  • Commit to working at Oak Street Health for 24 months following program completion
