Requirements

Candidates must be a board-certified Family Nurse Practitioner or Physician Assistant, licensed in Connecticut, and comfortable practicing the full spectrum of comprehensive care, including pediatrics and mental health. Familiarity with acute care, primary care, and behavioral health is required, as well as the ability to deliver text and video-based care remotely.

Responsibilities

The Advanced Practice Provider will deliver fully remote text and video-based acute care, encompassing preventive care, sick visits, and chronic condition management to patients. They will conduct video-based patient visits, support physicians with Epic inbox management, educate patients on appropriate treatments and care plans, prescribe medication and refills as needed, drive high-quality care with a focus on patient outcomes, and elevate remote care by providing compassionate and meaningful care.
