Requirements

  • Bachelor's level practitioner or high school graduate with 3 or more years of experience providing direct care to youth in a behavioral health agency or institutional setting
  • Must have and maintain a valid driver's license with an acceptable driving record
  • Must be able to exercise independent thinking and good judgment under all circumstances
  • Must be able to analyze work, set goals, develop plans and utilize time effectively and efficiently

Responsibilities

  • Provide individual and group recovery focused case management services
  • Collaborate with Acenda team members and act as a key partner to providing important input into our delivery of recovery services
  • Act as a liaison between providers and community agencies that provide supportive services
  • Provide education sessions, social service connections, and client transportation to support recovery needs
  • Organize and participate in Family Team Meetings/Child Family Team Meetings
  • Support adolescent and family with relevant school-related issues
  • Support connecting adolescent to work or extracurricular activities
  • Facilitate Adolescent/Family engagement and retention
  • May provide transportation to program participants for treatment-related activities
  • Function as part of treatment team
  • Documentation in agency’s Electronic Health Record & in CYBER
  • Performs other related duties as assigned
