Requirements

  • Professional registered nurse (RN) with CAP RN 2 designation (entry level for new employees meeting criteria or advancement from CAP RN 1)
  • Completion of CAP program requirements to maintain CAP RN 2 level or advance to CAP RN 3
  • Performs duties under direction of Program Manager (PM) and guidance from Educator, Preceptor, or Inpatient Services CAP RN 2,3,4, or 5 for first 3 months of employment
  • Familiarity with company, inpatient services program, and contracted provider emergency policies
  • Ability to work full-time, mainly covering hospital in Green Bay with occasional floating to Fox Valley area
  • Availability for shifts normally 5:30am-2pm or until treatments completed, with rotating on-call

Responsibilities

  • Perform ongoing, systematic collection and analysis of patient data pre-during-post treatment for assigned patients, document in medical record, adjust/modify treatment plan as indicated, and notify inpatient program manager, appropriately credentialed physician, patient hospital primary nurse, and others as needed
  • Recognize aspects and implications of patient status that vary from normal and report/collaborate with appropriate health team members (e.g., manager, physician, acute primary nurse)
  • Assess daily assigned patient care needs and collaborate with direct and ancillary patient care staff as needed
  • Direct and provide safe, effective patient care for assigned patients as prescribed for all modality-specific treatment procedures
  • Initiate and coordinate communication with FKC and Non-FKC dialysis providers and appropriate hospital personnel for continuity of patient care
  • Administer medications as prescribed and in accordance with contractual agreement
  • Ensure nephrology physician orders for assigned patients are received, entered, and in the Medical Record
  • Enter all treatment data into the designated medical record thoroughly, accurately, and timely
  • Implement, administer, monitor, and document patient's response to prescribed intradialytic transfusions, including notification of adverse reactions to physician and blood supplier
  • Initiate and assist with emergency response measures per policy
  • Delegate appropriate tasks to direct patient care staff (e.g., LVN/LPNs, Patient Care Technicians) and monitor for appropriate techniques and adherence to FKC policy/procedures
  • Promote infection control, equipment, and environmental safety
  • Assess, collaborate, and document patient/family's basic learning needs to provide initial and ongoing education
  • Serve as a leader, coach, mentor, and role model for new and incumbent employees by setting examples of appropriate behavior, work habits, and attitudes
  • May serve as a Preceptor to new employees, demonstrating, reinforcing, and encouraging appropriate attitudes and actions
  • Participate in decision-making, teaching, leadership functions, and quality improvement activities to enhance patient care outcomes and facility operations
  • Perform all duties in accordance with Fresenius Kidney Care and applicable contractual service policy, procedures, standards of nursing practice, state, and federal regulations
