Responsibilities

  • Performs ongoing assessment, planning, intervention, and evaluation of patient care
  • Documents patient data and adjusts treatment plans as needed
  • Provides initial and ongoing patient and family education
  • Recognizes and reports deviations from normal patient status
  • Directs and provides safe effective patient care
  • Assesses daily assigned patient care needs
  • Collaborates with direct and ancillary patient care staff
  • Communicates with Fresenius Kidney Care (FKC) and contracted facility personnel
  • Initiates and coordinates communication with FKC and Non-FKC dialysis providers
  • Administers and monitors intradialytic transfusions
  • Reports adverse reactions to physician and blood supplier
  • Administers medications as prescribed and documents justification and effectiveness
  • Initiates and assists with emergency response measures
  • Serves as a resource for the healthcare team
  • Participates in staff training and orientation
  • Provides specialized nursing care instruction to hospital/facility staff as assigned
  • Provides and coordinates clinically competent care for an assigned group of patients
  • Delegates tasks to other patient care staff
  • Participates in the kidney disease healthcare team
