Requirements

Candidates must have a Bachelor's degree in Nursing from an accredited university and hold a Compact Licensed RN or be eligible for other state licenses. A minimum of 2 years of direct clinical experience in an outpatient setting, Emergency Department/Triage, Complex Care, or PICU is required, along with experience with the pediatric population. Bilingual skills in English and Spanish are mandatory, as is comfortability communicating care with external partners and performing telephonic outreach to families. Experience with discharge planning and care coordination is preferred.

Responsibilities

The Transition of Care Nurse will develop relationships with pediatric offices, specialty practices, hospital discharge planners, private duty nurses, and community teams to support eligible patients and their families. They will outreach to Imagine Pediatric patients after hospital discharge to assist with care coordination, review discharge plans, and assess for post-discharge barriers. The nurse will leverage reporting to prioritize telephonic outreach, collaborate with the interdisciplinary team to ensure families have necessary post-discharge support, and coordinate care between patients and their providers for continuity. Other duties as assigned will also be performed.
