Requirements

  • Registered Nurse (RN) - active license in the state of practice from Board of Nursing upon hire
  • Visual acuity adequate to perform position duties
  • Ability to communicate effectively with others, hear, understand, and distinguish speech and other sounds
  • Ability to work night shift (6pm-6:30am), every third weekend and holiday, 72hrs/2wks
  • Preferred: 1-3 years in a long term care facility

Responsibilities

  • Provides leadership and day-to-day management; guides staff in meeting needs of residents; enforces facility staffing policies; acts as advocate for residents
  • Facilitates care conferences and multidisciplinary care planning process
  • Coordinates admissions, discharges, and transfers to and from the facility
  • Assures social service, activities, housekeeping, and nutrition meet needs of residents and clients
  • Ensures completion, with interdisciplinary team, of Resident Assessment Instrument (RAI) including MDS, RAPs, and Care Plans per facility, state, and federal guidelines
  • Enforces and maintains safe, secure, healthy resident care environment by adhering to infection-control policies, medication administration/storage procedures, controlled substance regulations, organization standards, policies, and legal regulations
  • Assists with data collection, audits for quality functions, performance improvement activities; develops and implements QAPI plan; leads Performance Improvement Committee as required
  • Assists in evaluating outcomes and processes to identify performance improvement opportunities
  • Provides resident care by reviewing physician prescriptions; communicating nursing care plan and pertinent information to health care team; coordinating with other providers; providing physical, emotional, psychological, spiritual support to residents, families, friends; conferring with physicians
  • Assures compliance with regulatory functions; interprets policies and objectives; collaborates with Compliance, Safety, Education, departments to educate staff, monitor compliance, recommend improvements
  • Directs and tracks orientation of new staff; provides follow-up; assures competencies and documentation are complete
  • Interviews, hires, develops, trains, retains employees; plans, assigns, leads work; appraises performance; rewards and coaches employees; addresses complaints and resolves problems
