Requirements

  • Registered Nurse (RN) license
  • Meets RN CAP 2 criteria or advancement from RN CAP 1
  • Ability to perform physical demands including heavy lifting, moving patients, and assisting with ambulation
  • Completion of CAP requirements to maintain or advance
  • Compliance with Fresenius Kidney Care policy, procedures, standards of nursing practice, state and federal regulations

Responsibilities

  • Perform ongoing, systematic collection and analysis of dialysis data for assigned patients, document in medical record, adjust treatment plans, and notify supervisor or physician as needed
  • Assess, collaborate, and document patient/family’s basic learning needs to provide initial and ongoing education
  • Direct and provide, in collaboration with patient, home care partner, and staff, safe and effective delivery of dialysis therapies
  • Administer medications as prescribed or per approved algorithms, documenting justification and effectiveness
  • Initiate or assist with emergency response measures
  • Serve as resource, leader, coach, mentor, and role model for employees
  • Ensure correct laboratory collection, processing, shipping, and reschedule missed collections
  • Identify expected outcomes, document and update nursing assessment and plan of care in collaboration with Interdisciplinary Team
  • Ensure patient awareness of transplant and treatment modality options
  • Assist in identification, evaluation, selection, and education of Home Dialysis candidates and Home Partners
  • Perform assessment of home environment and partner/family readiness for home dialysis
  • Train home dialysis patients and/or Home Partners on equipment and supplies via standardized program
  • Participate in education and quality improvement projects
  • Serve as Preceptor to new employees
  • Perform all other duties as assigned by Supervisor
