Requirements

Registered Nurse (RN) license
Meets RN CAP 2 criteria or advancement from RN CAP 1
Ability to perform physical demands including heavy lifting, moving patients, and assisting with ambulation
Completion of CAP requirements to maintain or advance
Compliance with Fresenius Kidney Care policy, procedures, standards of nursing practice, state and federal regulations

Responsibilities

Perform ongoing, systematic collection and analysis of dialysis data for assigned patients, document in medical record, adjust treatment plans, and notify supervisor or physician as needed
Assess, collaborate, and document patient/family’s basic learning needs to provide initial and ongoing education
Direct and provide, in collaboration with patient, home care partner, and staff, safe and effective delivery of dialysis therapies
Administer medications as prescribed or per approved algorithms, documenting justification and effectiveness
Initiate or assist with emergency response measures
Serve as resource, leader, coach, mentor, and role model for employees
Ensure correct laboratory collection, processing, shipping, and reschedule missed collections
Identify expected outcomes, document and update nursing assessment and plan of care in collaboration with Interdisciplinary Team
Ensure patient awareness of transplant and treatment modality options
Assist in identification, evaluation, selection, and education of Home Dialysis candidates and Home Partners
Perform assessment of home environment and partner/family readiness for home dialysis
Train home dialysis patients and/or Home Partners on equipment and supplies via standardized program
Participate in education and quality improvement projects
Serve as Preceptor to new employees
Perform all other duties as assigned by Supervisor
