Requirements

  • Registered Nurse (RN) license from the Board of Nursing - active in the state of practice upon hire
  • Visual acuity adequate to perform position duties
  • Ability to communicate effectively, hear, understand, and distinguish speech and other sounds
  • Preferred: Certified Hemodialysis Nurse (CHN), Certified Peritoneal Dialysis Nurse (CPDN), Certified Dialysis Nurse (CDN), or Specialty Certification from ASHI upon hire
  • Preferred: Dialysis experience or critical care experience
  • Preferred: 1-3 years nursing experience if functioning as a home dialysis nurse, plus an additional 3 months experience in each specific modality (hemodialysis or peritoneal dialysis) for self-care training
  • Ability to work primarily days with possible weekends/evenings/holidays (24hrs/2wks on Mon, Wed, Fri)

Responsibilities

  • Provides care and treatment to patients on maintenance dialysis within the scope of nursing practice
  • Performs and maintains knowledge and proficiency in technical skills for dialysis treatments
  • Provides oversight and delegation to department staff
  • Understands and ensures compliance related to water room equipment operation and function
  • Complies with established departmental policies and procedures, scope of care, performance improvement activities, safety, environmental, and infection control standards
  • Documents in the patient record according to departmental policy/procedure
  • Promotes and maintains documentation and an environment compliant with state, federal, and regulatory agencies (including CMS/ESRD-Renal Network) and Quality Assurance/Process Improvement efforts
  • Coordinates care of patients in collaboration with Medical Director, Nephrologist, patients, families, other nursing staff, and ancillary departments
  • Acts as a patient/family advocate and collaborates with patient, family, and health care providers in providing patient care
  • Identifies patient/family learning needs, develops, implements, and evaluates an educational plan for patients
  • Provides patient education related to dialysis process, procedures, and care/management of dialysis accesses
  • Home dialysis nurses provide major portions of self-care and home dialysis training for patients and/or caregivers
