Requirements

  • Registered Nurse (RN) license
  • Entry-level designation into the Clinical Advancement Program (CAP 1)
  • Ability to perform all essential functions under the direction of the Supervisor with guidance from the Educator, Preceptor, or in collaboration with another Registered Nurse
  • Capability to handle physical demands including direct patient care with heavy lifting
  • Completion of CAP requirements to advance
  • Works primarily in clinic setting with initial home visits accompanied by another staff member; various shifts and work hours

Responsibilities

  • Performs ongoing, systematic collection and analysis of dialysis data for assigned patients, documents in medical record, adjusts treatment plans as indicated, and notifies Supervisor or physician as needed
  • Assesses, collaborates, and documents patient/family’s basic learning needs; provides initial and ongoing education to patients and family
  • Directs and provides, in collaboration with patient, home care partner, direct and ancillary staff, all aspects of safe and effective delivery of dialysis therapy
  • Administers medications as prescribed or per approved algorithms, documents justification and effectiveness
  • Initiates or assists with emergency response measures
  • Serves as resource for health care team, participates in staff training and orientation of new staff
  • Ensures correct laboratory collection, processing, shipping; reschedules missed or insufficient collections
  • Identifies expected outcomes, documents and updates nursing assessment and plan of care through collaboration with Interdisciplinary Team
  • Ensures patient awareness of transplant and treatment modality options
  • Assists in identification, evaluation, selection, and education of Home Dialysis candidates and Home Partners
  • Performs assessment of Home Dialysis candidate’s home environment, partner/family readiness and ability
  • Trains Home Dialysis patients and/or Home Partners on safe, effective operation and maintenance of equipment and supplies via organized training program
  • Participates in decision-making, teaching, leadership functions, and quality improvement activities
  • Communicates patient-related issues to physician as needed
  • Performs all other duties as assigned by Supervisor
