Registered Nurse (RN) licensure
Ability to travel to all four locations (Enola, Camp Hill, Middletown, Newberry) as needed
Availability for full-time hours: 8:00am - 4:30pm Monday to Friday, with light on-call duties

Responsibilities

Participate in decision-making, teaching, leadership, and quality improvement activities as a member of the nephrology health care team to enhance patient care outcomes and home therapies program operations
Identify and evaluate candidates for the Home Therapies Program, including assessment of patients’ home environment if needed
Perform assessment and identify barriers of the Home Dialysis candidate’s home environment and caretaker readiness and ability to perform dialysis treatments in the home
Admit new patients to the Home Therapies Program using the appropriate processes
Coordinate patient care including assessment, planning, intervention, and evaluation for an assigned group of hemodialysis patients
Assess and manage patients’ response to home dialysis training and treatment therapy by following prescribed predetermined protocols
Analyze dialysis data for Home Therapy patients and document and adjust or modify the treatment plan as needed
Provide initial and ongoing education to patient and family on home dialysis treatment and the operation and maintenance of all home therapies equipment
Ensure patient awareness related to transplant and treatment modality options
Administer medications as prescribed or in accordance with approved algorithm(s), and document appropriate medical justification and effectiveness
Initiate or assist with emergency responses
Conduct initial visits to patients' homes to assess home therapy readiness, with regular training and follow-up visits primarily in the clinic environment
Perform all duties in accordance with Fresenius Kidney Care policy, procedures, standards of nursing practice, state, and federal regulations
