The role primarily works in the clinic setting and conducts initial home visits with another staff member present, functioning as an onsite Patient Educator and Case Manager.

Required skills involve compliance with policies and regulations, patient assessment, education on ESRD and equipment operation, and communication with physicians.

The role is part of the Home Therapies dialysis health care team in a clinic setting, involving rotations with other licensed staff, participation in staff meetings and training, and monthly CQI meetings to support quality enhancement and patient satisfaction goals.

Strong candidates should have RN licensure and experience in dialysis patient education, home therapy training, patient assessment, and quality improvement activities, with the ability to work various shifts and collaborate in a team environment.
