Responsibilities

  • Assesses patient’s knowledge of diet and kidney disease
  • Provides patient-specific detailed education regarding nutritional status, adequacy measures, fluid status, and nutrition-related lab parameters
  • Completes nutritional assessments and participates in interdisciplinary comprehensive assessments
  • Participates in the development of individualized comprehensive plans of care
  • Calculates diet prescriptions according to established guidelines
  • Completes progress notes on chronic dialysis patients
  • Maintains accurate and complete documentation in medical records
  • Provides ongoing nutrition assessment and outcome-oriented nutrition counseling
  • Identifies malnourished patients and collaborates with the team to implement interventions
  • Reviews patient albumin levels and body weight monthly
  • Monitors adherence to nutrition therapy and adjusts therapy as needed
  • Implements plan of care to address issues impacting patient goals
  • Collaborates with the interdisciplinary team on Urea Kinetic Modeling recommendations
  • Collaborates with the interdisciplinary team on management of mineral metabolism, anemia, and renal bone disease
