Responsibilities

Conduct nutritional assessments of patients, evaluating their knowledge of diet and kidney disease
Provide tailored, detailed education to patients, families, and caregivers regarding the relationship between diet and kidney disease, and its impact on treatment
Participate in the development of individualized comprehensive plans of care, in collaboration with the interdisciplinary team
Calculate diet prescriptions according to established standards of practice
Complete progress notes on chronic dialysis patients, maintaining accurate and complete medical record documentation
Provide ongoing nutrition assessment and outcome-oriented counseling to patients
Identify malnourished patients and collaborate with the team to implement appropriate interventions
Review patient albumin levels and body weight monthly
Implement the plan of care with patient/family/caregiver involvement, adjusting therapy as needed
Collaborate with the interdisciplinary team on Urea Kinetic Modeling recommendations and management of mineral metabolism, anemia, and renal bone disease
Support the organization's Quality Enhancement Program (QEP) and Quality Assessment and Performance Improvement (QAI) activities
