Responsibilities

  • Conduct nutritional assessments of patients, evaluating their knowledge of diet and kidney disease
  • Provide tailored, detailed education to patients, families, and caregivers regarding the relationship between diet and kidney disease, and its impact on treatment
  • Participate in the development of individualized comprehensive plans of care, in collaboration with the interdisciplinary team
  • Calculate diet prescriptions according to established standards of practice
  • Complete progress notes on chronic dialysis patients, maintaining accurate and complete medical record documentation
  • Provide ongoing nutrition assessment and outcome-oriented counseling to patients
  • Identify malnourished patients and collaborate with the team to implement appropriate interventions
  • Review patient albumin levels and body weight monthly
  • Implement the plan of care with patient/family/caregiver involvement, adjusting therapy as needed
  • Collaborate with the interdisciplinary team on Urea Kinetic Modeling recommendations and management of mineral metabolism, anemia, and renal bone disease
  • Support the organization's Quality Enhancement Program (QEP) and Quality Assessment and Performance Improvement (QAI) activities
