Requirements

  • Registered Dietitian licensure
  • Knowledge of diet and kidney disease relationship and impact on treatment
  • Ability to assess patient's learning needs and provide appropriate education
  • Skills in nutritional assessment and calculating diet prescriptions per standards of practice
  • Proficiency in completing progress notes and maintaining documentation per FMCNA policies, Federal, and state regulations
  • Competence in monitoring lab parameters (e.g., albumin, body weight, urea kinetic modeling, mineral metabolism, anemia, iron status)
  • Ability to collaborate with interdisciplinary teams on patient care plans, QAI/QEP activities, and process improvements
  • Experience in patient/family/caregiver counseling and education, including Pre-ESRD coordination

Responsibilities

  • Assess patient’s knowledge of diet and kidney disease; provide tailored education on nutritional status, adequacy measures, fluid status, and lab parameters
  • Complete nutritional assessments and participate in interdisciplinary comprehensive assessments and care plan development
  • Calculate diet prescriptions and provide ongoing nutrition assessment and counseling to achieve effective nutritional status
  • Identify malnourished or at-risk patients; collaborate on interventions with interdisciplinary team
  • Review monthly albumin levels and body weights; implement and adjust plans of care with patient/family involvement
  • Monitor adherence/response to nutrition therapy; document barriers and implement solutions
  • Collaborate on Urea Kinetic Modeling, mineral metabolism, renal bone disease, anemia, nutritional/psychosocial status, vascular access, modality, rehabilitation, and sodium/volume control
  • Report nutrition QAI results; participate in interdisciplinary QAI program and staff meetings
  • Coordinate Pre-ESRD education and refer for nutritional supplements as needed
  • Review nutrition-related lab results; counsel patients/caregivers; recommend and communicate treatment changes to physicians, staff, and care facilities
