Requirements

  • Registered Dietitian licensure
  • Knowledge of diet and kidney disease relationship and impact on treatment

  • Proficiency in interdisciplinary collaboration and comprehensive care planning per Federal and state guidelines
  • Competence in documentation per FMCNA policies, Federal, and state regulations
  • Ability to identify malnourished patients and at-risk patients, recommending interventions
  • Experience with monitoring lab parameters (e.g., albumin, body weight, Kt/V, mineral metabolism, anemia, iron status)
  • Skills in Urea Kinetic Modeling, nutrition counseling, and therapy adjustment
  • Familiarity with FMCNA algorithms, QEP, QAI, and process improvement activities

Responsibilities

  • Assess patient’s knowledge of diet and kidney disease; provide tailored education to patient/family/caregiver on nutritional status, adequacy, fluid status, and lab parameters
  • Complete nutritional assessments and participate in interdisciplinary comprehensive assessments and care plan development
  • Calculate diet prescriptions and complete progress notes on chronic dialysis patients per FMCNA guidelines
  • Maintain accurate documentation in medical records
  • Provide ongoing nutrition assessment, counseling, and therapy monitoring/adjustments to achieve effective nutritional status
  • Review monthly albumin levels and body weights; implement plan of care with patient/family/caregiver involvement
  • Identify malnutrition risks and collaborate on interventions/resources
  • Collaborate on Urea Kinetic Modeling, mineral metabolism, renal bone disease, anemia, iron, nutritional/psychosocial status, vascular access, modality, rehabilitation, and sodium/volume control
  • Adhere to FMCNA policies; report on nutrition QAI results and participate in QAI program
  • Coordinate Pre-ESRD education and work with Case Manager for patient education and supplement referrals
  • Review nutrition-related lab results; counsel patient/caregiver; recommend treatment changes; communicate with physicians, staff, and external facilities
  • Participate in staff meetings (e.g., general staff, quality improvement)
