Requirements

  • Familiarity with FMCNA guidelines, Federal and state regulations, and licensure guidelines
  • Proficiency in nutritional assessment, diet prescription calculation, and lab parameter interpretation (e.g., albumin, body weight, Kt/V, mineral metabolism, anemia/iron status)
  • Ability to function as a member of an interdisciplinary healthcare team

Responsibilities

  • Assess patient’s knowledge of diet and kidney disease and provide education appropriate to patient’s learning needs regarding the relationship of diet and kidney disease and impact on treatment
  • Provide patient-specific detailed education to patient/family/caregiver on nutritional status, adequacy measures and results, fluid status, and nutrition-related lab parameters
  • Complete nutritional assessment and participate in interdisciplinary comprehensive patient assessments
  • Participate with interdisciplinary team in developing written, individualized comprehensive plan of care per Federal and state guidelines
  • Calculate diet prescription according to standard of practice
  • Complete progress notes on all chronic dialysis patients per FMCNA guidelines
  • Maintain complete and accurate documentation in medical record per FMCNA policies and Federal/state regulations
  • Provide ongoing nutrition assessment and outcome-oriented nutrition counseling to help patients achieve and sustain effective nutritional status
  • Identify malnourished patients or those at risk for malnutrition and collaborate with interdisciplinary team on interventions/resources/solutions
  • Review each patient’s albumin level and body weight at least monthly
  • Implement plan of care with involvement of patient/family/caregiver
  • Monitor adherence/response to nutrition therapy, adjust as needed, document barriers to goals, and implement plan to address issues
  • Collaborate with interdisciplinary team on Urea Kinetic Modeling recommendations to achieve physician’s Kt/V prescription
  • Collaborate with interdisciplinary team on management/recommendations for mineral metabolism/renal bone disease, anemia/iron status, nutritional/psychosocial status, vascular access, modality, rehabilitation status, and sodium/volume control
  • Adhere to FMCNA algorithm and medical record policies; report nutrition QAI results and participate in interdisciplinary QAI program
  • Collaborate/coordinate with team on Pre-ESRD education as appropriate
  • Work with Case Manager to coordinate patient education plan and identify candidates for physician referral for nutritional supplements per disease management agreements
  • Review nutrition-related lab results, counsel patient/caregiver, formulate actions, recommend treatment changes to team, and communicate results/changes to physicians, facility staff, and nursing facility providers
  • Participate in pertinent staff meetings
