Responsibilities

Utilize evidence-based nutrition practices to provide dietary counseling and support to patients with severe mental illness in a clinical setting
Conduct behavioral assessments to understand patients' health and behavioral needs, and coordinate tailored health coaching plans
Implement coaching plans using behavior change principles to identify barriers and develop strategies to overcome them
Support behavioral interventions related to dietitian-led dietary modification plans
Monitor and evaluate patient interventions and outcomes, adapting and modifying as needed
Lead daily and weekly group activities incorporating light movement and physical wellness into care plans
Coordinate with psychiatrists, therapists, and support teams to provide feedback on patient goal attainment and clinical issues
Participate in research projects, working with patients and research teams to implement them
Assess clients’ physical health and develop comprehensive health plans focusing on lifestyle changes (diet, exercise, sleep, financial health, relationship health, treatment compliance)
Conduct individual and group coaching sessions to help patients maintain treatment goals, reduce stress, improve mindfulness, nutrition, medication adherence, and increase physical exercise
