Requirements

  • Functions as a member of the interdisciplinary healthcare team
  • Supports the FMCNA commitment to the Quality Enhancement Program (QEP) and Quality Assessment and Performance Improvement (QAI) activities
  • Actively participates in process improvement activities
  • Completes nutritional assessment and participates in interdisciplinary comprehensive assessments
  • Participates with the interdisciplinary team in developing a written, individualized comprehensive plan of care per Federal and state guidelines
  • Calculates diet prescription according to standard of practice
  • Completes progress notes on all chronic dialysis patients per FMCNA guidelines
  • Maintains complete and accurate documentation in medical record per FMCNA policies and Federal/state regulations
  • Reviews each patient’s albumin level and body weight at least monthly
  • Adheres to FMCNA algorithm and medical record policies
  • Reports on nutrition QAI results and participates in the interdisciplinary QAI program
  • Collaborates and coordinates with team members in provision of Pre-ESRD education as appropriate
  • Works with Case Manager to coordinate plan of care for patient education and identifies candidates for nutritional supplements per disease management agreements
  • Reviews nutrition related lab results, counsels patient/caregiver, formulates appropriate action, recommends treatment changes, and communicates with physicians/staff/providers

Responsibilities

  • Provides nutritional services for in-center and home patients to maximize nutritional status and improve clinical outcomes
  • Assesses patient’s knowledge of diet and kidney disease and provides education appropriate to learning needs on diet-kidney disease relationship and treatment impact
  • Provides patient-specific detailed education to patient/family/caregiver on nutritional status, adequacy measures/results, fluid status, and nutrition-related lab parameters
  • Provides ongoing nutrition assessment and outcome-oriented nutrition counseling to help patients achieve and sustain effective nutritional status
  • Identifies malnourished patients or those at risk and collaborates with interdisciplinary team on interventions/resources/solutions
  • Implements plan of care with involvement of patient/family/caregiver
  • Monitors adherence and response to nutrition therapy, adjusts therapy as needed, documents reasons for unmet goals, and implements plan of care to address issues
  • Collaborates with interdisciplinary team on Urea Kinetic Modeling recommendations to achieve physician’s Kt/V prescription
  • Collaborates with interdisciplinary team on management/recommendations for mineral metabolism/renal bone disease, anemia/iron status, nutritional status, psychosocial status, vascular access, modality, rehabilitation status, and sodium/volume control
  • Participates in pertinent staff meetings
