Registered Dietitian credentials
Knowledge of diet, kidney disease, and its impact on treatment processes
Ability to assess patient's nutritional status, knowledge, and learning needs
Proficiency in calculating diet prescriptions according to standards of practice
Skills in interdisciplinary collaboration for comprehensive patient care planning
Competence in monitoring lab parameters (e.g., albumin, body weight, Kt/V, mineral metabolism, anemia)
Experience with nutritional assessment, counseling, and education for patients, families, and caregivers
Familiarity with FMCNA guidelines, Federal and state regulations, QEP, QAI, and process improvement activities
Ability to document accurately in medical records and complete progress notes

Responsibilities

Assess patient’s knowledge of diet and kidney disease; provide tailored education on diet's relationship to kidney disease and treatment
Deliver patient-specific education on nutritional status, adequacy measures, fluid status, and nutrition-related lab parameters
Complete nutritional assessments and participate in interdisciplinary comprehensive assessments
Develop written, individualized comprehensive plans of care with the interdisciplinary team per Federal and state guidelines
Calculate diet prescriptions and complete progress notes on chronic dialysis patients per FMCNA guidelines
Maintain complete and accurate documentation in medical records per policies and regulations
Provide ongoing nutrition assessment, outcome-oriented counseling, and implement plans of care with patient/family/caregiver involvement
Identify malnourished or at-risk patients; collaborate on interventions, resources, or solutions
Review monthly albumin levels and body weights; monitor adherence/response to therapy and adjust as needed
Document reasons for unmet goals and implement plans to address barriers
Collaborate on Urea Kinetic Modeling (Kt/V), mineral metabolism, renal bone disease, anemia, iron, nutritional/psychosocial status, vascular access, modality, rehabilitation, and sodium/volume control
Adhere to FMCNA algorithms and medical record policies; report on nutrition QAI results and participate in QAI programs
Coordinate Pre-ESRD education and work with Case Manager on care plans, nutritional supplements, and physician referrals
Review nutrition-related lab results; counsel patients/caregivers; recommend/formulate actions and treatment changes
Communicate lab results and recommendations to physicians, facility staff, and providers in nursing facilities
Participate in pertinent staff meetings (e.g., general staff, quality improvement)
