  • Proficiency in maintaining complete and accurate documentation in medical records per FMCNA policies, Federal, and state regulations
  • Skills in ongoing nutrition assessment, counseling, and identifying malnutrition risks
  • Ability to monitor adherence/response to nutrition therapy and adjust as needed
  • Knowledge of Urea Kinetic Modeling (Kt/V), mineral metabolism, renal bone disease, anemia, iron status, psychosocial status, vascular access, modality, rehabilitation, and sodium/volume control
  • Familiarity with FMCNA algorithms, QAI/QEP programs, and process improvement activities
  • Open to new grads and candidates with less than 1 year of clinical experience

Responsibilities

  • Assess patient’s knowledge of diet and kidney disease and provide tailored education on diet's relationship to kidney disease and treatment
  • Provide detailed education to patient/family/caregiver on nutritional status, adequacy measures, fluid status, and nutrition-related lab parameters
  • Complete nutritional assessments and participate in interdisciplinary comprehensive assessments and care plan development per Federal/state guidelines
  • Calculate diet prescriptions according to standard of practice
  • Complete progress notes on all chronic dialysis patients per FMCNA guidelines
  • Maintain complete and accurate documentation in medical records per policies and regulations
  • Provide ongoing nutrition assessment and outcome-oriented counseling to achieve/sustain effective nutritional status
  • Identify malnourished/at-risk patients and collaborate on interventions/resources
  • Review each patient’s albumin level and body weight at least monthly; implement plan of care with patient/family/caregiver involvement
  • Monitor adherence/response to nutrition therapy, adjust as needed, document barriers, and implement solutions
  • Collaborate on Urea Kinetic Modeling recommendations to achieve Kt/V prescription
  • Collaborate on management/recommendations for mineral metabolism, renal bone disease, anemia/iron, nutritional/psychosocial status, vascular access, modality, rehabilitation, and sodium/volume control
  • Adhere to FMCNA algorithm and medical record policies; report on nutrition QAI results and participate in QAI program
  • Collaborate/coordinate on Pre-ESRD education and work with Case Manager on care plans, nutritional supplements referrals
  • Review nutrition-related lab results, counsel patient/caregiver, formulate actions, recommend treatment changes, and communicate with physicians/staff/providers
