Responsibilities

Provide behavioral healthcare management to patients, monitoring and managing care in alignment with a holistic approach using various treatment modalities as patients progress through recovery phases
Co-manage a panel of complex patients through behavioral health assessments, diagnosis, treatment recommendations, treatment planning, clinical treatment, medication management, and follow-up planning
Develop individualized treatment plans early in the engagement and stabilization phase, focused on addressing specific psychosocial and medical needs that might limit recovery
Provide continuous, regular consultations to patients navigating recovery, both in-person and occasionally virtually
Liaise with patient care teams and collaborate closely with behavioral health providers to ensure an integrated, cohesive approach to care
Act as a patient advocate, helping members navigate health and social services systems, addressing problems, answering questions, gathering/providing information and advice, and connecting to resources and community
Initiate, establish, and maintain relationships rooted in trust and rapport with patients and their families
Address medical needs to mitigate negative impacts of social determinants of health
Partner with clinical staff, peer specialists, primary care providers, and community partners in a collaborative, shared decision-making model
