Responsibilities

  • Provide behavioral healthcare management to patients, monitoring and managing care in alignment with a holistic approach using various treatment modalities as patients progress through recovery phases
  • Co-manage a panel of complex patients through behavioral health assessments, diagnosis, treatment recommendations, treatment planning, clinical treatment, medication management, and follow-up planning
  • Develop individualized treatment plans early in the engagement and stabilization phase, focused on addressing specific psychosocial and medical needs that might limit recovery
  • Provide continuous, regular consultations to patients navigating recovery, both in-person and occasionally virtually
  • Liaise with patient care teams and collaborate closely with behavioral health providers to ensure an integrated, cohesive approach to care
  • Act as a patient advocate, helping members navigate health and social services systems, addressing problems, answering questions, gathering/providing information and advice, and connecting to resources and community
  • Initiate, establish, and maintain relationships rooted in trust and rapport with patients and their families
  • Address medical needs to mitigate negative impacts of social determinants of health
  • Partner with clinical staff, peer specialists, primary care providers, and community partners in a collaborative, shared decision-making model
