Responsibilities

Provide behavioral healthcare management to Amae Health patients, monitoring and managing care in alignment with a holistic approach using various treatment modalities as patients progress through recovery phases
Co-manage a panel of complex patients through behavioral health assessments, diagnosis, treatment recommendations, treatment planning, clinical treatment, medication management, and follow-up planning
Develop individualized treatment plans early in the engagement and stabilization phase, focused on addressing specific psychosocial and medical needs that limit recovery
Provide continuous, regular consultations to patients navigating recovery, both in person and occasionally virtually
Liaise with patient care teams and collaborate closely with behavioral health providers, primary care providers, peer specialists, and community partners to ensure integrated, cohesive care
Act as a patient advocate, helping members navigate health and social services systems, addressing problems, answering questions, gathering and providing information and advice, and connecting patients to resources and the community
Initiate, establish, and maintain trusting relationships with patients and their families
Address medical needs to mitigate negative impacts of social determinants of health
