Physician - Generalist (Palliative Care)

Position Overview

Included Health is seeking a compassionate and dedicated Physician Generalist, fellowship-trained in Palliative Care, to join our Care & Case Management (CCM) Program. In this vital role, you will support members throughout their healthcare journey, providing holistic, patient-centered care through remote consultations and case management. Working as part of a multidisciplinary team, you will develop personalized care plans, offer expert clinical guidance, and empower members to make informed health decisions, particularly when managing complex conditions like cancer. You will collaborate with local treating teams, coordinate resources, and ensure seamless care transitions to improve health outcomes. Additionally, you will contribute to program improvement by analyzing clinical trends and member data. Requirements

  • Licensure & Certification:
    • Active medical license and Board Certification in Family Medicine (ABMS or AOA) or Internal Medicine (ABIM).
    • Additional certification or relevant experience in Palliative Care is required. Candidates without formal certification but with relevant experience will be considered.
  • Experience:
    • 3 years of Family Medicine or Internal Medicine residency or equivalent.
    • 2 years of post-residency experience providing direct patient care.
    • Candidates with 3+ years of post-residency experience and the ability to treat all age populations will be prioritized.
  • State Licensure:
    • Willingness to become and maintain licensure in multiple states to meet program needs.
    • IMLC state residency is important.
    • Preference for licenses in Indiana, Alabama, Georgia, Mississippi, Missouri, Oklahoma, South Carolina, and Tennessee.
  • Physical/Cognitive Requirements:
    • Prompt and regular attendance at assigned work location.
    • Ability to remain seated in a stationary position for prolonged periods.
    • Eye-hand coordination and manual dexterity to operate keyboard, computer, and other office equipment.
    • Occasional exertion of up to 20 lbs of force (e.g., lifting a computer/laptop).
    • Ability to work effectively with leadership, employees, and members.

Responsibilities

  • Provide holistic, patient-centered care through remote consultations and case management.
  • Develop personalized care plans for members.
  • Offer expert clinical guidance and empower members in their health decisions.
  • Manage complex conditions, including cancer, throughout the care journey.
  • Collaborate with local treating teams and coordinate resources.
  • Ensure seamless care transitions to improve health outcomes.
  • Analyze clinical trends and member data to contribute to program improvement.

Shift Obligations

  • Hours: 8 AM - 5 PM, Monday - Friday (local time).
  • Estimated Start Date: Between March - April 2025, depending on licensing status and notice periods.

Pay & Benefits

  • New Hire Base Salary Range (US): $203,330 - $287,210 + Benefits + Equity.
