Responsibilities

  • Evaluates assigned patients, gathering data from sources such as patient records, interviews, observation, and team members
  • Develops and implements individualized treatment programs for patients, reviews plan of care with patient, and instructs patient, family, caregivers in home programs or exercises
  • Uses selected activities/tasks to promote health, achieve treatment goals, and modify interventions based on ongoing evaluation
  • Provides direct therapy services using functional settings and strategies, coordinated with rehabilitation, medical, and nursing care plans
  • Prepares documentation including evaluation results, individualized treatment plans, progress notes, and other reports
  • Communicates effectively with patient, family, physician, and interdisciplinary team members regarding status, progress, and needs; attends care conferences
  • Establishes rapport and works cooperatively with interdisciplinary team members (including family/physician)
  • Enhances professional growth through educational programs, literature, in-services, workshops, etc
  • Oversees physical therapy assistants, techs, and students to ensure safe patient care and smooth operations
  • Maintains knowledge of proper use of equipment for patient care
  • Coordinates and delivers daily physical therapy services according to principles of physical therapy and department objectives
