Responsibilities

Evaluates assigned patients, gathering data from sources such as patient records, interviews, observation, and team members
Develops and implements individualized treatment programs for patients, reviews plan of care with patient, and instructs patient, family, caregivers in home programs or exercises
Uses selected activities/tasks to promote health, achieve treatment goals, and modify interventions based on ongoing evaluation
Provides direct therapy services using functional settings and strategies, coordinated with rehabilitation, medical, and nursing care plans
Prepares documentation including evaluation results, individualized treatment plans, progress notes, and other reports
Communicates effectively with patient, family, physician, and interdisciplinary team members regarding status, progress, and needs; attends care conferences
Establishes rapport and works cooperatively with interdisciplinary team members (including family/physician)
Enhances professional growth through educational programs, literature, in-services, workshops, etc
Oversees physical therapy assistants, techs, and students to ensure safe patient care and smooth operations
Maintains knowledge of proper use of equipment for patient care
Coordinates and delivers daily physical therapy services according to principles of physical therapy and department objectives
