Requirements

Current and valid PHRN license
At least 1 year of bedside/clinical experience
Familiarity with MDS (Minimum Data Set) is an advantage
Strong knowledge of prehospital patient care protocols, standards, and regulatory requirements
Excellent attention to detail and analytical skills
Effective communication and interpersonal abilities
Ability to work independently and collaboratively in a team-oriented environment
Commitment to continuous learning and professional development
Must be residing within GMA (Greater Manila Area)
Willing to work on shifting schedule

Responsibilities

Conducting comprehensive reviews of patient care documentation in accordance with established standards, protocols, and regulatory requirements
Maintaining the quality and accuracy of prehospital patient records
Collaborating closely with clinical staff, managers, and other stakeholders to identify areas for improvement and provide feedback for ongoing education and training
Complete and submit accurate Minimum Data Set (MDS) assessments
Develop and update individualized care plans for residents
Ensure assessments align with Medicare/Medicaid and state regulations
Work with nurses, therapists, and healthcare teams for quality patient care
Maintain precise documentation to support reimbursement and compliance
