Responsibilities
- Individuele begeleiding op maat bieden aan ouder wordende cliënten met uiteenlopende zorgvragen en zelfstandigheidsniveaus
- Een fijne sfeer creëren in de algemene ruimtes
- Inschatten wanneer over te nemen en wanneer aan te moedigen om zelf te doen (bijv. helpen bij opstarten van de dag of gesprekken voeren)
- Werken op verschillende woonlocaties in Volendam (zelfstandig op Brederodestraat, Henegouwen, Jan Platstraat; met collega's op 24-uurslocaties zoals Helena Smitsstraat en Boelenspark)
- Ondersteunen bij dagelijks leven: samen koken, medicatie aanreiken, verpleegtechnische handelingen (diabeteszorg, epilepsie, stomazorg)
- Zorgplan opstellen en uitvoeren
- Nauw samenwerken met collega-begeleiders, familie, netwerk, gedragsdeskundige, regiebegeleider, huisarts en AVG
- Verantwoordelijk zijn voor het opstellen en uitvoeren van acties rondom een aantal cliënten
