Responsibilities

  • Ontwikkelen en uitvoeren van ondersteuningsplannen voor logees, inclusief regelmatige evaluatie in samenwerking met gedragsdeskundige en team
  • Logees verwelkomen bij aankomst en zorgen voor warme, rustige overgang
  • Organiseren van activiteiten passend bij interesses en behoeften (bijv. wandelen, spelletjes, knutselen, uitstapjes)
  • Begeleiden bij dagelijkse routines en ADL-taken (Activiteiten van de Dagelijkse Leven)
  • Eerste aanspreekpunt zijn voor logees, hun netwerk en collega's
  • Voortgang van zorg bewaken en begeleiding afstemmen op behoeften en doelen
  • Collega’s coachen voor juiste zorg en ondersteuning
  • Huiselijke en veilige omgeving creëren met structuur, plezier, betrokkenheid bij eetmomenten en netheid
  • Nachtpresence waarborgen voor rust en veiligheid
  • Samenwerken met team, kennis delen en bijdragen aan visie en invulling van logeerlocaties
