  • Proficiency in Salesforce CRM (familiarity with Salesforce and EMR systems a plus)

  • Ability to work overtime when required
  • Availability for full-time hours Monday-Friday 9am-6pm EST

Responsibilities

  • Place new and existing patients with Fresenius Kidney Care (FKC) through excellent sales and customer service skills
  • Perform case management while serving as the primary liaison with referral sources, clinics, and discharge planners
  • Obtain chair times, financial clearance, and medical clearance in a timely manner to ensure an expedited admissions process
  • Serve as the primary contact for initial administration of patient referral calls and communications regarding patient admission to clinics (chronic and/or acute) and home programs
  • Coordinate the admissions process from initial request for admission to patient placement in preferred clinic
  • Ensure all intake data for admissions requests are collected, completed, and communicated according to standard operating procedures and in compliance with regulatory requirements
  • Obtain necessary demographic, financial, and clinical information through contact/interview with referral provider, patient, or patient representative to complete admissions process
  • Assist other teams in processing referrals/calls during transient or busy periods
  • Interview patient, patient’s representative, or referral source to obtain personal and financial data for eligibility determination
  • Utilize call tracking and admissions program systems (e.g., Salesforce) for accurate documentation of calls, maintaining and updating with current data, status, and details
  • Complete preliminary paperwork and standard admissions forms for efficient processing
  • Coordinate cross-functionally with multiple departments and resources within FKC to place patients
  • Promote efficiency of admissions process by answering phones and assisting callers professionally and timely, handling escalations, and collaborating with supervisor/manager
  • Work with insurance verification teams to ensure financial clearance for placement in FKC facility
  • Communicate with clinics for prompt patient scheduling and forward necessary documents/records to receiving facilities timely
  • Collaborate with facility staff and management to resolve issues impacting patient placement
