Requirements

Functions as part of the dialysis health care team under direct supervision of a licensed nurse (Home Therapies registered nurse - HT RN)
Compliance with organization policies, procedures, training, and state/federal regulations
Performs all duties and responsibilities of PCT III HT under direct supervision of HT RN, as permissible per state laws, regulations, and policies
Supports Quality Enhancement Program (QEP), CQI activities, patient satisfaction, and process improvement for QEP goals

Responsibilities

Reinforces home hemodialysis (HHD) and/or peritoneal dialysis (PD) training as permitted by law
Identifies and welcomes patients, obtains vital signs, weights, and gathers information for Clinic Visit Intake form
Builds and maintains effective communication and collaboration with patients, care partners, HT RN, staff, providers, and team members
Obtains treatment records from patients/systems, reviews for completion, reports missing/abnormal data to HT RN
Assists with electronic home treatment/medication/oral nutritional supplement reconciliation, reports missing data/orders to HT RN
May assist HT RNs with home visits and provides home support as applicable/permissible
Prepares supplies for prescribed laboratory testing, prepares specimens for shipping
Assists with tracking/shipping transplant-related lab specimens, reinforces transplant education
Completes opening/closing daily chores and documents appropriately
Disinfects dialysis machines, chairs, tables, equipment between/routinely patient visits
Prepares exam/training room for home dialysis training/treatment/visits
Stocks exam, training, treatment, storage rooms with supplies
Develops/follows process for supply management to maximize use/minimize waste
Schedules patient appointments, contacts patients/caregivers for dates/times/reminders
Supports patient registration and use of connected health
Assists patients/care partners with supplies/inventory ordering/management
Maintains logs and tracking
May order/track clinic/office supplies as directed
Participates in medical records, infection control, other audit processes as directed
Actively collaborates in staff huddles/meetings
Helps collect documentation for patient admission, prepares/maintains electronic/hard-copy patient chart as directed
Performs other clerical duties (answering phones, faxing, email, provider contact, etc.)
Reinforces HHD training/education with HT training RN
Sets up, tears down, programs, tests, disinfects HHD machine as directed
Troubleshoots HHD equipment/connectivity issues
Prepares dialysis equipment/supplies for HHD treatment as directed
Evaluates dialysis access before treatment, documents findings, reports abnormal to HT RN (as applicable/directed)
Initiates HHD treatment, monitors/documents patient status, vital signs, machine data
Performs/documents safety checks during treatment per CMS/facility policy intervals
Reports observations (description cuts off here)
