Requirements

  • Functions as part of the dialysis health care team under direct supervision of a licensed nurse, in compliance with organization policies, procedures, training, and state/federal regulations
  • Performs all duties and responsibilities of PCT III HT under direct supervision of Home Therapies registered nurse (HT RN), as permissible per state laws, regulations, and policies
  • Supports organization's Quality Enhancement Program (QEP) and CQI activities, including patient satisfaction and process improvements

Responsibilities

  • Reinforces home hemodialysis (HHD) and/or peritoneal dialysis (PD) training as permitted by law
  • Identifies and welcomes patients, obtains vital signs, weights, and gathers information for Clinic Visit Intake form
  • Builds and maintains effective communication and collaboration with patients, care partners, HT RN, staff, providers, and team members
  • Obtains treatment records from patients/systems, reviews for completion, and reports missing/abnormal data to HT RN
  • Assists with electronic home treatment/medication/oral nutritional supplement reconciliation and reports missing data/orders to HT RN
  • May assist HT RNs with home visits and provides home support as applicable and permissible
  • Prepares supplies for prescribed laboratory testing, prepares specimens for shipping, and assists with tracking/shipping transplant-related specimens; reinforces transplant education
  • Completes opening/closing daily chores and documents as appropriate
  • Disinfects dialysis machines, chairs, tables, and other equipment between patient visits and routinely
  • Prepares exam/training room for home dialysis training, treatment, or visits; stocks rooms with supplies
  • Develops or follows process for supply management to maximize use and minimize waste
  • Schedules patient appointments, contacts patients/caregivers for dates, times, and reminders
  • Supports patient registration and use of connected health
  • Assists patients/care partners with supplies/inventory ordering and management
  • Maintains logs and tracking; may order/track clinic/office supplies as directed
  • Participates in medical records, infection control, and other audit processes as directed
  • Actively collaborates in staff huddles or meetings
  • Helps collect documentation for patient admission, prepares/maintains electronic/hard-copy patient charts as directed
  • Performs other clerical duties (answering phones, faxing, email, provider contact, etc.)
  • Reinforces HHD training and education with HT training RN
  • Sets up, tears down, programs, tests, and disinfects HHD machine as directed
  • Troubleshoots HHD equipment and connectivity issues
  • Prepares dialysis equipment and supplies for HHD treatment as directed
  • Evaluates dialysis access before treatment, documents findings, reports abnormal findings to HT RN
  • Initiates HHD treatment, monitors/documents patient status, vital signs, and machine data
  • Performs and documents safety checks during treatment per CMS and facility policy
  • Reports observations (description cuts off here)
