Requirements

Functions under direct supervision of a licensed nurse
Compliance with organization policies, procedures, training, and state/federal regulations
Knowledge of dialysis therapy, including Online Clearance Monitoring (OLC), Adequacy Monitoring Program (AMP), Urea Kinetic Modeling (UKM), disease process/access, vascular access cannulation, heparin administration (as delegated or allowed by state law)
Ability to perform setup and operation of hemodialysis machines
Ability to calculate and enter dialysis machine programming for Ultrafiltration (UF) goal, treatment time, and UF modeling
Ability to ensure safety alarms are enabled (e.g., Narrow Venous Limits 160)
Ability to monitor patients, evaluate vascular access, obtain vital signs/weights, initiate/discontinue treatments, and document accurately
Ability to assist in maintaining a safe and clean working environment
Participation in Quality Enhancement Program (QEP), CQI activities, process improvement, and patient satisfaction initiatives
Ability to train applicable direct patient care staff

Responsibilities

Assist in patient education on adequacy measures (OLC, AMP, UKM) and disease process/access
Welcome patients, inquire about wellbeing, evaluate vascular access for patency, cannulate access, administer heparin as allowed
Obtain pre- and post-treatment vital signs, weights, and vascular access evaluations
Initiate dialysis per prescribed orders (blood flow QB, dialysate flow QD)
Monitor patients during treatment, evaluate intradialytic problems, intervene as prescribed/directed
Obtain vital signs before reinfusion, discontinue treatment per procedures, evaluate patient (standing/sitting BP), achieve hemostasis, apply dressings
Evaluate patient prior to discharge, perform/record pre/post evaluations
Calculate and enter UF goal, treatment time, UF modeling; ensure safety alarms enabled
Record vital signs, treatment parameters on Hemodialysis Treatment Sheet with initial identification
Document patient treatment info (Hemodialysis Treatment Sheet, progress notes); report significant changes to RN/supervisor
Observe patients, conduct machine safety checks, report changes/unusual findings
Document interventions for unusual patient status and patient response
Monitor vascular access (arterial/venous pressures), intervene as needed, document/report findings
Obtain prescribed lab testing, prepare specimens
Clean blood spills per policy
Prepare/organize supplies/equipment to prevent wastage
Monitor self-care patients under RN supervision
Assist in training direct patient care staff
