Requirements

  • Functions under direct supervision of a licensed nurse
  • Compliance with organization policies, procedures, training, and state/federal regulations
  • Knowledge of dialysis therapy, including Online Clearance Monitoring (OLC), Adequacy Monitoring Program (AMP), Urea Kinetic Modeling (UKM), disease process/access, vascular access cannulation, heparin administration (as delegated or allowed by state law)
  • Ability to perform setup and operation of hemodialysis machines
  • Ability to calculate and enter dialysis machine programming for Ultrafiltration (UF) goal, treatment time, and UF modeling
  • Ability to ensure safety alarms are enabled (e.g., Narrow Venous Limits 160)
  • Ability to monitor patients, evaluate vascular access, obtain vital signs/weights, initiate/discontinue treatments, and document accurately
  • Ability to assist in maintaining a safe and clean working environment
  • Participation in Quality Enhancement Program (QEP), CQI activities, process improvement, and patient satisfaction initiatives
  • Ability to train applicable direct patient care staff

Responsibilities

  • Assist in patient education on adequacy measures (OLC, AMP, UKM) and disease process/access
  • Welcome patients, inquire about wellbeing, evaluate vascular access for patency, cannulate access, administer heparin as allowed
  • Obtain pre- and post-treatment vital signs, weights, and vascular access evaluations
  • Initiate dialysis per prescribed orders (blood flow QB, dialysate flow QD)
  • Monitor patients during treatment, evaluate intradialytic problems, intervene as prescribed/directed
  • Obtain vital signs before reinfusion, discontinue treatment per procedures, evaluate patient (standing/sitting BP), achieve hemostasis, apply dressings
  • Evaluate patient prior to discharge, perform/record pre/post evaluations
  • Calculate and enter UF goal, treatment time, UF modeling; ensure safety alarms enabled
  • Record vital signs, treatment parameters on Hemodialysis Treatment Sheet with initial identification
  • Document patient treatment info (Hemodialysis Treatment Sheet, progress notes); report significant changes to RN/supervisor
  • Observe patients, conduct machine safety checks, report changes/unusual findings
  • Document interventions for unusual patient status and patient response
  • Monitor vascular access (arterial/venous pressures), intervene as needed, document/report findings
  • Obtain prescribed lab testing, prepare specimens
  • Clean blood spills per policy
  • Prepare/organize supplies/equipment to prevent wastage
  • Monitor self-care patients under RN supervision
  • Assist in training direct patient care staff
