Requirements

  • Functions under direct supervision of a licensed nurse
  • Compliance with organization policies, procedures, training, and state/federal regulations
  • Knowledge of dialysis therapy, including Online Clearance Monitoring (OLC), Adequacy Monitoring Program (AMP), Urea Kinetic Modeling (UKM), disease process/access, vascular access cannulation, heparin administration (as delegated or allowed by state law)
  • Ability to perform setup and operation of hemodialysis machines
  • Ability to calculate and enter dialysis machine programming for Ultrafiltration (UF) goal, treatment time, and UF modeling
  • Ability to ensure safety alarms are enabled (e.g., Narrow Venous Limits 160)
  • Ability to monitor patients, evaluate vascular access, obtain vital signs/weights, initiate/discontinue treatments, manage intradialytic problems, and perform interventions as prescribed
  • Accurate and timely documentation skills (e.g., Hemodialysis Treatment Sheet, progress notes)
  • Observation and machine safety check skills
  • Ability to assist in training direct patient care staff

Responsibilities

  • Assist in patient education on adequacy measures (OLC, AMP, UKM) and disease process/access
  • Welcome patients, inquire about wellbeing, evaluate vascular access for patency, cannulate access, administer heparin as allowed
  • Obtain pre- and post-treatment vital signs, weights, and vascular access evaluations
  • Initiate dialysis per orders (QB, QD), monitor response, evaluate intradialytic problems, intervene as prescribed
  • Obtain vital signs before reinfusion, discontinue treatment, evaluate patient (standing/sitting BP), achieve hemostasis, apply dressings
  • Evaluate patient prior to discharge, perform/record pre/post evaluations
  • Calculate/enter UF goal, treatment time, UF modeling; ensure safety alarms enabled
  • Record vital signs/treatment parameters on Hemodialysis Treatment Sheet with initials
  • Document patient treatment info, complete sheets/progress notes
  • Report significant changes/unusual findings to RN/supervisor
  • Conduct machine safety checks, observe patients, document interventions/responses
  • Monitor vascular access (arterial/venous pressures), intervene/report as needed
  • Obtain lab specimens, clean blood spills per policy
  • Prepare/organize supplies/equipment to prevent wastage
  • Monitor self-care patients under RN supervision
  • Assist in training direct patient care staff
  • Maintain safe/clean environment, support QEP/CQI activities, participate in process improvements
