Requirements

Candidates must be board-certified in Family or Internal Medicine and possess at least 3 years of post-residency work experience. A minimum of 2 years of experience in an in-person primary care setting and prior experience with EPIC EMR are required. Candidates must also have a clean background and medical malpractice history, be willing to work a rotating schedule of 7 days on and 14 days off, and be licensed in NY, WI, MN, or IA, or willing to obtain these licenses. Experience with virtual visits is also required.

Responsibilities

Physicians will provide high-quality preventative, acute, and chronic care in a fully remote, virtual setting, managing a dedicated panel of patients. They will refer patients to specialists or acute care services when clinically indicated, work on population health initiatives, and support patients post-discharge to reduce hospital readmissions. Physicians will also practice continuous self-evaluation to ensure adherence to clinical guidelines and best practices.
