  • Entry level designation into RN CAP 2 or advancement from RN CAP 1
  • Meets RN CAP 2 criteria
  • Ability to perform ongoing, systematic collection and analysis of patient data pre-during-post hemodialysis treatment
  • Ability to assess, collaborate, and document patient/family learning needs
  • Ability to direct and provide hemodialysis therapy in collaboration with staff
  • Ability to administer medications as prescribed or per approved algorithms
  • Ability to initiate or assist with emergency response measures
  • Serves as resource, leader, coach, mentor, and role model
  • Ensures correct laboratory collection, processing, and shipping
  • Identifies expected outcomes and updates nursing assessment/plan of care
  • Ensures patient awareness of transplant and treatment modality options
  • Participates in education and quality improvement projects
  • May serve as Preceptor to new employees
  • Required to complete CAP requirements to maintain or advance
  • Ability to perform heavy lifting, moving patients, assisting with ambulation, frequent prolonged standing, bending, and moving equipment up to 200 lbs. (with assistance), lifting solutions up to 30 lbs. as high as 5 feet
  • Works in health care facility environment with air temperature control and moderate noise

Responsibilities

  • Provision and coordination of clinically competent care including assessment, planning, intervention, and evaluation for assigned patients
  • Delegation of appropriate tasks to direct patient care staff (RNs, LVN/LPNs, Patient Care Technicians)
  • Participates in decision-making, teaching, leadership functions, and quality improvement activities
  • Performs all duties in accordance with Fresenius Kidney Care policy, procedures, standards of nursing practice, state, and federal regulations
  • Performs essential functions under direction of Supervisor and guidance from Clinical Educator, Preceptor, or in collaboration with other RNs
  • Documents patient data in medical record, makes adjustments to treatment plan, and notifies Team Leader, Charge Nurse, Supervisor, or Physician as needed
  • Provides initial and ongoing education to patients and family
  • Directs and provides all aspects of daily safe and effective hemodialysis therapy delivery
  • Documents medication administration, justification, and effectiveness
  • Serves as resource, leader, coach, mentor, and role model for employees
  • Ensures correct laboratory collection, processing, shipping, and reschedules missed collections
  • Documents and updates nursing assessment and plan of care through Interdisciplinary Team collaboration
  • Ensures patient awareness related to transplant and treatment modality options
  • Participates in education and quality improvement projects at facility and area level
  • Performs all other duties as assigned by Supervisor
