Requirements

  • Registered Nurse (RN) license and entry-level designation into Clinical Advancement Program (CAP 2) or advancement from RN CAP 1
  • Ability to perform ongoing, systematic collection and analysis of patient data pre-during-post hemodialysis treatment
  • Capability to assess, collaborate, and document patient/family learning needs for education
  • Proficiency in directing and providing hemodialysis therapy in collaboration with staff
  • Skills in administering medications per prescriptions or algorithms with documentation
  • Ability to initiate or assist with emergency response measures
  • Competence in laboratory collection, processing, shipping, and rescheduling
  • Knowledge to identify expected outcomes, update nursing assessments, and collaborate with Interdisciplinary Team
  • Ability to ensure patient awareness of transplant and treatment options
  • Physical ability for heavy lifting, moving patients, assisting with ambulation, frequent prolonged standing, bending, and occasionally moving equipment up to 200 lbs. or lifting solutions up to 30 lbs
  • Completion of CAP requirements to maintain or advance position

Responsibilities

  • Provision and coordination of clinically competent care including assessment, planning, intervention, and evaluation for assigned patients
  • Delegation of appropriate tasks to direct patient care staff (RNs, LVN/LPNs, Patient Care Technicians)
  • Participation in decision-making, teaching, leadership, and quality improvement activities
  • Documentation of patient data in medical records and adjustments to treatment plans, notifying leaders or physicians as needed
  • Provision of initial and ongoing education to patients and family
  • Daily provision of safe and effective hemodialysis therapy to assigned patients
  • Administration of medications with justification and effectiveness documentation
  • Serving as resource, leader, coach, mentor, and role model for employees
  • Ensuring correct laboratory procedures and rescheduling missed collections
  • Updating nursing assessment and plan of care through Interdisciplinary Team collaboration
  • Participation in education and quality improvement projects
  • May serve as Preceptor to new employees
  • Perform all other duties as assigned by Supervisor
