Registered Nurse (RN) license in good standing
RN CAP 2 designation or advanced from RN CAP 1 (criteria not specified)
Knowledge of hemodialysis procedures and protocols
Ability to assess, plan, intervene, and evaluate patient care
Ability to delegate tasks appropriately
Strong communication and interpersonal skills
Ability to work effectively as part of a multidisciplinary team

Responsibilities

Performs ongoing, systematic collection and analysis of patient data pre-during-post hemodialysis treatment
Documents patient data and makes adjustments to the treatment plan as needed
Notifies Team Leader, Charge Nurse, Supervisor, or Physician as required
Performs assessments and collaborates on patient/family learning needs, providing initial and ongoing education
Directs and provides daily hemodialysis therapy to assigned patients in collaboration with other staff
Administers medications as prescribed or in accordance with approved algorithms, documenting justification and effectiveness
Initiates or assists with emergency response measures
Serves as a resource, coach, mentor, and role model for new and incumbent employees
Ensures correct laboratory collection, processing, and shipping procedures are followed
Identifies expected outcomes and updates the nursing assessment and plan of care
Participates in education and quality improvement projects
May serve as a Preceptor to new employees
Participates in decision-making, teaching, leadership functions, and quality improvement activities
Ensures patient awareness related to transplant and treatment modality options
