Requirements

  • Registered Nurse (RN) license in good standing
  • RN CAP 2 designation or advanced from RN CAP 1 (criteria not specified)
  • Knowledge of hemodialysis procedures and protocols
  • Ability to assess, plan, intervene, and evaluate patient care
  • Ability to delegate tasks appropriately
  • Strong communication and interpersonal skills
  • Ability to work effectively as part of a multidisciplinary team

Responsibilities

  • Performs ongoing, systematic collection and analysis of patient data pre-during-post hemodialysis treatment
  • Documents patient data and makes adjustments to the treatment plan as needed
  • Notifies Team Leader, Charge Nurse, Supervisor, or Physician as required
  • Performs assessments and collaborates on patient/family learning needs, providing initial and ongoing education
  • Directs and provides daily hemodialysis therapy to assigned patients in collaboration with other staff
  • Administers medications as prescribed or in accordance with approved algorithms, documenting justification and effectiveness
  • Initiates or assists with emergency response measures
  • Serves as a resource, coach, mentor, and role model for new and incumbent employees
  • Ensures correct laboratory collection, processing, and shipping procedures are followed
  • Identifies expected outcomes and updates the nursing assessment and plan of care
  • Participates in education and quality improvement projects
  • May serve as a Preceptor to new employees
  • Participates in decision-making, teaching, leadership functions, and quality improvement activities
  • Ensures patient awareness related to transplant and treatment modality options
